A NEW PARISH AFFILIATION FORM MUST BE SIGNED EVERY YEAR in preparation for the 2017-18 School Year Tuition
To apply for Tuition Assistance you must first attain a SIGNED PARISH AFFILIATION FORM.
To do this you must first: download the appropriate Parish Affiliation form – either:
- the St. Anne form, because you are registered at St. Anne Parish, or
- an Out of Parish form, meaning you are registered at a church other than St. Anne’s Church.
You will find the forms on Sycamore-document- Parish Affiliation forms. Or come to the school office and pick up an Affiliation form in the office. Take this form to your parish and have it signed by your pastor or his representative. Once signed, the form must be brought to the school office in order for FAIR to process your application.
The FAIR window for Tuition Assistance will open February 2, 2017.
