The In-Parish Affiliation Form is for Shrine of St. Anne parishioners and the Out-Of-Parish Affiliation Form is for families that are registered at any other parish.

We recommend that you print out the appropriate Parish Affiliation Form now to take it to your Parish, get it signed by your Pastor, pick it up once it is signed, and bring it to the school office. That way you will have one important piece of Registration ready to go long before Registration begins.

The In-Parish Affiliation Form is for Shrine of St. Anne parishioners and the Out-Of-Parish Affiliation Form is for families that are registered at any other parish.

An important reminder:  Our Pastors wanted us to be sure that parents are well informed on their requirements for signing off on Parish Affiliation, whether it is In-Parish or Out-Of-Parish Affiliation. Those requirements are:

  • The family has been registered in the parish for at least six (6) months.
  • The family verifiably contributes, according to their means, regularly to the financial support of the parish.
  • The family attends Mass regularly and is involved in the activities, organizations and/or programs at the parish.

Some questions you may be asked by your pastor this year before he signs your Affiliation Form for general reasons and due to the Covid shutdown:

  • How have you supported your parish with financial contributions (as a suggestion, if you give cash in the offertory at your church, be sure to put it in your family envelope so your parish is aware of your giving).
  • How did your family observe the Sabbath during the Covid shutdown?
  • What ways does your family support your parish beyond financial contributions?
  • Include a picture of your family with your Affiliation Form for your pastor to be sure he is putting the correct name with families.  It is also a good idea to be sure you have introduced your family to your pastor.

These requirements are prudent and reasonable. All parishes financially support our Catholic Schools because our schools are an integral part of the mission of our Church, but parishes have a responsibility to be sure that the family asking for Affiliation is committed to the formation of their child(ren).

Tuition Assistance

Affording a Catholic education is often difficult, especially for practicing Catholics who are contributing members of their parishes. This is why the Shrine of St. Anne Catholic School offers tuition assistance to fully-affiliated families who are enduring financial hardship.
